Does anyone have a Microsoft Excel template that shows buys, sells and profit percent to track my trades?

I have one (Actually I have 2).

I use one for stocks, and another for options (just learning these). if you send me a PM, I can forward you one or both. The second one is more simple, and can be used for stocks too.

I use a very simple one, no macro's or anything fancy.  Just figured it may be a good idea for tax purposes.  I'd like to see what other people use as well.
